Hospital Passports

 

We have been working together with Playwise to develop hospital passports to support our young children and pupils to be able to effectively access the medical support they need. This involves communicating essential information to those that are providing their care.

PlayWise Learning CIC (Community Interest Company) has been established to offer parents and carers of young disabled children, and the professionals that support them, with a wide range of services.  This will help reduce the impact of disability and SEND (Special Educational Needs and Disability) and support families on their journey. PlayWise offers new, innovative and bespoke interventions for families either at home, in our wonderful community hub in central Sutton or in the child’s own learning setting. PlayWise supports children aged 0-8 years.

PlayWise works in partnership with parents and carers to support all aspects of a child’s learning and development. The aim of our services are to empower parents, carers and educators, to enhance the child’s learning and development by creating meaningful opportunities and increasing positive outcomes for the child and family. We carry this approach into our bespoke community groups and courses, and work in the same nurturing way in school and nursery environments, making sure everyone has everything needed to give the SEND child maximum opportunities to flourish.
